      Transitional Care

      At Kimball Health Services, we understand there are times when a patient no longer needs the acute care they receive in a hospital – yet they may not feel strong enough to return home. For those patients, we offer a comforting alternative. Our Swing Bed Transitional Care Program is designed to provide patients with an individualized, in-hospital care and physical rehabilitation program to meet the patient’s goals to improve and return to a more independent setting.

      Swing Bed is a special Medicare program designed to help a person recover from surgery or illness close to their home. Swing Bed care allows patients to transition from acute care to skilled rehabilitation at Kimball Health Services. This ensures continuity of care for patients needing nursing care by allowing them to stay in a local, familiar environment with the same doctors, nurses and therapists they already know and trust. Our Swing Bed care provides the same services as skilled nursing units at other healthcare facilities.

      Swing Bed patients receive nursing care daily. Therapy care is received daily and over the weekends as needed. The care plan includes treatment to restore the ability to perform activities of daily living such as walking, climbing stairs, dressing and bathing. Our staff also works with patients to provide activities that they would normally do at home.

      Kimball Health Services staff members will meet with patients and their families to discuss the plan of care and rehabilitation goals. Kimball Health Services’ interdisciplinary team will continuously monitor progress and evaluate new goals until they determine the patient is ready for discharge.

      Our social worker / swing bed coordinator will assist you in making appropriate arrangements regarding services and equipment to ensure your safe return home. Therapists may also educate you and your family or caregiver on techniques and practical adjustments to make when you return home.

      Does Medicare / Medicaid pay for swing bed services?

      Swing bed care is often covered by Medicare, Medicaid and many private insurance plans after meeting certain requirements. For more information, contact our Social Services Department at (308) 235-1952, ext. 159.
