Your medical record is a legal record that you have the right to inspect, review and receive a copy of. To request a copy of your medical record, submit a completed authorization form electronically, by fax, mail, email or in person. Staff will make every effort to fill your request as soon as possible; however it may take up to 30 days from receipt of a properly executed request. Verification of identity by photo ID is required. Acceptable forms of identification include driver’s license or other official photo ID. If you are requesting copies for someone other than yourself, you will need to provide legal documentation verifying the legal guardianship, power of attorney, executorship or next of kin relationship of decedent.
An authorization form may be picked up and/or dropped off at the Kimball Health Services Registration Desk in the main entrance, 505 S Burg Street in Kimball, between the hours of 7 am to 5:00 p.m.

The Kimball Health Services Patient Portal offers a secure and easy way to access your personal health history, screening results, and vital information about your care. The portal is simple to use, available around the clock and completely private.
The patient portal allows access to your medical record to view 1) the providers that have given you care; 2) health problems; 3) allergies; 4) immunizations; 5) medications; 6) in-house lab test results; 7) vital signs and 8) radiology reports. These can be downloaded and shared with other providers.
During admission, a Kimball Health Services staff member will record your email address to create your access to the patient portal. An automatic email is sent to you to invite you to access your patient portal. If you receive care in our clinics and the hospital, you will need to create two (2) profiles – one for your clinic records and a second account for your hospital records. Nebraska immunization records can be obtained by visiting the Nebraska State Immunization System at dhhs.ne.gov/nesiis
Wyoming residents may obtain a copy of their immunization record by contacting our Pine Bluffs Health Clinic Office and requesting a copy from the Wyoming Immunization Registry.
Radiology imaging information can be obtained from the Radiology Department.

The Admissions Office is just through the west doors of the hospital. Making sure we have complete and accurate patient information begins with admission. To help verify information and for accurate billing, please bring a copy of your most current insurance card and driver’s license or photo id.
Kimball Health Services is a smoke-free environment. This supports our commitment to providing a safe and healthy workplace while promoting the health and wellbeing of our employees and patients. Smoking is not allowed anywhere inside or outside of our facilities.
If you speak a language other than English, language assistance and TDD services are available to you free of charge.
Our Social Services Department is available to assist you with any concerns you may have regarding adjustment to illness, hospitalization, or financial concerns related to services at our clinic, hospitalization or illness. Social Services can also help arrange medical services or assist with other needs that you may require after dismissal from our facility.
Kimball Health Services has a certified Senior Health Insurance Information Program (SHIIP) counselor available for unbiased information on Medicare education and assistance with Medicaid.
